Prerequisites
Have an account with access to the questionnaire activity on the platform.
Steps
Click on “New”, select the activity “Questionnaire” and then “Begin” the activity.
The request is created. The request number appears at the top of the page.
Two options are possible.
Option 1: The professional sends the invitation to complete the questionnaire to the user or their caregiver from the invitation tab.
Option 2: The professional completes the questionnaire with the user's collaboration from the questionnaire tab.
