Prerequisite
Having an account with access to the questionnaire activity on the platform.
Steps
The questionnaire can be completed:
-In real-time: the professional completes it in the presence of the user
-In deferred time: the user visits the guest portal after having received the invitation to complete the questionnaire

Completing the questionnaire
The professional clicks on "Start" to begin the questionnaire.
The box on the left indicates the current stage of the questionnaire.
The number of responses answered / the total number of responses are displayed.
The blue dot indicates the page currently being viewed.
The green dot indicates which sections have been completed in full.
The white dot indicates the sections to be filled in.
Complete the questionnaire. Choose from the available answers.
Use the "Previous" and "Next" buttons to navigate between the different sections of the questionnaire.
The question box will appear in white once the answer has been entered.
Use the "Reset" button at any time before you "submit the form" to change the answer provided.
Submit the questionnaire
Guest portal: Completing the questionnaire offline
For the user or their caregiver to complete the questionnaire, the professional must send the invitation. The designated person will receive an email notification inviting them to answer the questions from the guest portal. If necessary, the professional can send a reminder or edit the delivery details.
Inviting the user
The professional sends the invitation by completing the required information.
They choose the recipient: the user themselves or a caregiver. They enter the email address, validate the consent and send the message. : if the option chosen is to send the consent to a caregiver, additional information will be requested.
If necessary, before sending, the professional may extend the time limit for the user's response.
By default, the user will have 14 days to answer the questionnaire.
An automated reminder is sent after 7 days if the questionnaire has not been submitted. If needed, the professional can also send a reminder e-mail invitation.
The professional can also "Edit" the delivery details.
They can choose to modify the following:
1-The recipient
2-The email address
3-The submission deadline
By clicking "Send Invitation” the professional sends the updated invitation to the selected recipient.
Receiving the invitation
The user receives an email with the link and clicks to proceed.
The link in the email is for single use only.
The questionnaire is available in both languages.
Answering questions
The user accesses the guest portal.
The questionnaire is displayed.
They answer the questions.
Questions can be reset at any time before the questionnaire is submitted.
The user can also choose to save the questionnaire and return to it later.
ATTENTION:
The user accesses the guest portal via a single-use hyper-secure link. They receive the new link directly by email automatically OR by requesting it from the portal, using a link already in use. Once the questionnaire is completed, the user submits it.
